Helios Care (formerly Catskill Area Hospice and Palliative Care), which serves Delaware, Otsego, and Schoharie Counties, has received a grant from the Mother Cabrini Health Foundation to help provide palliative care to additional patients. The grant will fund the Helios Care CHOICES program, which provides person-centered, in-home care and care coordination. CHOICES focuses on managing symptoms, defining personal preferences, and developing plans to best address all of a patient’s care needs.

The grant will fund palliative care for patients who have a chronic disease or face a serious illness and are frequent visitors to the Emergency Department or admissions to the hospital.
